Weight loss surgery — also known as bariatric surgery — is the most effective long-term treatment for severe obesity. It is generally considered for adults who:

  • Have a BMI of 40 or above, OR
  • Have a BMI of 35 or above with significant weight-related health conditions (type 2 diabetes, sleep apnoea, hypertension, joint disease)
  • Have made sustained attempts at non-surgical weight loss without adequate results

Surgery is not a quick fix — it requires lifelong commitment to dietary changes, supplementation, follow-up, and lifestyle modification. However, for the right patient, it can be life-changing.

Types of Bariatric Surgery Available in Melbourne

Sleeve Gastrectomy

The sleeve gastrectomy is currently the most commonly performed bariatric procedure in Australia. Approximately 80% of the stomach is removed laparoscopically, leaving a narrow "sleeve" shape. This reduces food intake and significantly lowers ghrelin (the hunger hormone).

  • Advantages: Highly effective; often resolves type 2 diabetes
  • Considerations: More complex surgery; requires close long-term nutritional monitoring

Adjustable Gastric Band

The lap band places a silicone band around the upper stomach to create restriction. It is reversible and adjustable, but requires regular follow-up for band adjustments.

  • Expected weight loss: 40–50% of excess weight
  • Advantages: Reversible; no cutting of stomach or intestines
  • Considerations: Requires ongoing adjustments; lower long-term success than sleeve or bypass

Single Anastomosis Duodenal Switch (SADS)

A newer procedure combining elements of the sleeve gastrectomy with intestinal bypass. Reserved for patients with severe obesity or metabolic disease.

The Surgical Journey: What to Expect

1. Referral and initial consultation

Your GP refers you to a bariatric surgeon. Dr Chong can provide referrals and pre-operative medical optimisation.

2. Pre-operative assessment

Blood tests, sleep study, dietitian review, psychology assessment, and medical optimisation.

3. Surgery

Most bariatric procedures are performed laparoscopically (keyhole) under general anaesthetic, with a 1–3 day hospital stay.

4. Recovery

Most patients return to normal activities within 2–4 weeks. Dietary progression from liquids to solids occurs over several weeks.

5. Long-term follow-up

Regular reviews with your bariatric GP, surgeon, and dietitian are essential. Blood tests for nutritional deficiencies should be performed every 6–12 months for life.
